By submitting this form, I certify that I am the authorized user of this credit/debit card and will not dispute any payment of fees under this agreement. I understand and agree that the charge will come from the Cruise Line directly and am authorizing Heavens 2 Betsy Travel, LLC to process my credit/debit card referenced above in the amount indicated and agree to the Heavens 2 Betsy Travel, LLC's Terms and Conditions of booking including suppliers' and agency's cancellation and refund policies. I certify that I have verified that the stated amount, and all information contained in the proposal I received is accurate. This authorization/charge will be manually applied by Heavens 2 Betsy Travel, LLC to my reservation on the Cruise Line's booking site. If there are any issues, I will be contacted. 

I confirm that I am authorized to act on behalf of myself and all travelers on the same booking. 

I authorize Heavens 2 Betsy Travel, LLC to credit any amounts that have been charged in error.

I understand that I am responsible for all amounts, including any associated costs of recovery:

• If I close the credit/debit card I provide to Heavens 2 Betsy Travel, LLC;
• If I charge-back any fees that were validly charged under this Authorization; 
• If my credit/debit card expires; or
• If I have provided an incorrect account number or have insufficient funds at any time (including declinations of validly charged amounts).

In any such event, I agree Heavens 2 Betsy Travel, LLC is entitled to withhold all Services until I satisfy all payment deficiencies. Heavens 2 Betsy Travel, LLC has no obligation to refund any amounts previously paid by me as a result of suspension or termination of services.I understand that it is my responsibility to verify the ability to travel for every traveler in my party, including but not limited to restrictions enforced by countries I am visiting related to criminal records or unpaid child support, as well as COVID-19 restrictions.

All guests are solely responsible to possess and carry and all legal documentation, including health related declaration to be permitted to travel to and in the country planned and booked by the Heavens 2 Betsy Travel. Vendors nor travel agents are not responsible for verification of any requirements.

I understand all of the terms and conditions regarding this booking including cancellation policies, applicable penalties, and the availability of travel insurance.  Any Plan to Travel Fees paid are separate from the cost of travel and are non-refundable.

If I am using a debit card with a daily spending limit, it is my responsibility to contact my bank to give them permission to authorize the transaction. If they require the name of the vendor and you are unsure as to who that is, please contact Betsy.

The parties agree that this agreement may be electronically signed. The parties agree that the electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.

By signing my name below and submitting this form, I agree to the terms provided in this document and the Terms and Conditions of Heavens 2 Betsy Travel, LLC, and I understand this is a legally binding agreement.
